Referral Process

To refer a young adult to the Vocational Immersion Program (VIP) complete and submit the documents listed below.
For young adults, 18–21 years of age, currently enrolled in a secondary or post-secondary school and receiving services through an IEP or 504 plan:
- VIP Referral Form (to be completed by counselor, vo-tech teacher, any professional with a knowledge of student's vocational experience)
- Current IEP or 504 Plan
- Current RR (within past two years)
- VIP Parent Form (to be completed by parent/guardian)
- VIP Participant Form (to be completed by student)
For young adults, 18-24 years of age, not currently enrolled in a secondary or post-secondary school and have a disability:
- VIP Referral Form (to be completed by counselor, vo-tech teacher, any professional with a knowledge of applicant's vocational experience)
- VIP Parent Form (to be completed by parent/guardian)
- VIP Participant Form(to be completed by applicant)
- Any relevant work-related assessments, vocational profiles, etc.
Financial assistance may be available to eligible families. Complete and submit Financial Information Form to request financial assistance.
